Team rosters must have a minimum of 3 and a maximum of 5 players. All players must have a tournament or league rating as of 2/1/2011 (highest of the two used for players with both). Maximum rating difference between players on team cannot exceed 300 points (500 points for teams with rating average above 2300 or below 1000). Youth division players must be 14 or younger as of 1/1/2011.

In consideration of being permitted to participate in any way in the LATTF table tennis league, hereinafter called “League”, I, for myself and/or minor child, my heirs, personal representatives or assigns, do:

1. Waiver: Hereby release, waive, discharge, and covenant not to sue the Los Angeles Table Tennis Federation of California, its officers, employees, and agents from liability from any and all claims resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in the League.

2. Assumption of Risks: Fully understand that participation in table tennis activities and the League carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. These risks include serious bodily injury, permanent disability, paralysis and death. I fully accept and assume all such risks and all responsibility for any losses, costs and damages incurred as a result of participation in the League.

3. Indemnification and Hold Harmless: Agree to indemnify and hold the Los Angeles Table Tennis Federation and its agents harmless from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of my involvement in the League and to reimburse them for any such expenses incurred.

4. Severability: Expressly agree that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of the State of California and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

5. Image and Likeness: Hereby give my full permission for the use of my and/or my minor child’s name, picture, image, likeness, actions, voice, or other personally identifiable information, including any photographs, audio, video recordings, or other materials captured in conjunction with the League. I grant the Los Angeles Table Tennis Federation the right to use such photographs, audio, video recordings, or other materials for news, promotional, or other purposes, and I waive all rights of privacy or compensation, which I may have in connection with such use. I waive the right to inspect or approve the use of my and/or my minor child’s name, picture, image, likeness, actions, voice, or other personally identifiable information and hereby release, indemnify and hold harmless the Los Angeles Table Tennis Federation and its agents from and against any claims, damages or liability arising from such use.

6. Acknowledgment of Understanding: Have read this waiver of liability, assumption of risk, and
indemnity agreement, fully understand its terms, and understand that I am giving up substantial
rights, including my right to sue. I acknowledge that I am consenting to the agreement freely and voluntarily, and intend by my consent to be a complete and unconditional release of all
liability to the greatest extent allowed by law.
